Download the claim file, fill it in and send it back with the rest of required documents

2. Collect a list of documents:

Information as evidence in support of the claim, including information to show that the bodily injury is a result of an accident
Permission to obtain medical reports or records needed from any doctor or medical consultant who has treated you
